These guidelines outline responsibilities for staff providing one-to-one (“sitter”) monitoring for patients who may be a danger to themselves or others or whose behavior is unpredictable. Before beginning, the sitter must receive a report from the nurse and ask questions to fully understand the patient’s risks and required precautions.<br /><br />Sitters may talk with alert patients but must not share personal information or answer questions about the patient’s care or treatment. Any patient requests to walk, shower/bathe, or participate in activities (e.g., drawing, cards) require prior approval from nursing staff.<br /><br />The sitter must scan the environment for potential hazards and alert nursing staff about items that could be used for self-harm or harm to others, such as wires, pens/pencils, strings, plastic bags, or medical equipment. The sitter should maintain a safe distance while staying close enough to intervene quickly, including accompanying the patient to the bathroom and monitoring from the doorway with the door slightly open.<br /><br />The patient must never be left alone; if the sitter needs to step away, nursing staff must provide coverage. The sitter must immediately notify nursing staff if the patient expresses unsafe thoughts (e.g., wanting to hurt themselves or others), shares concerning information, becomes excessively agitated, attempts self-harm or harm to others using any object (including linens or plastic bags), or if the sitter feels threatened.<br /><br />Sitters should not provide requested items unless specifically directed by the nurse, since access may be restricted for safety. Finally, the sitter must document the patient’s location and behavior every 15 minutes on the observation flow sheet.
